The symptoms and complications of prostatitis

 

1.Pathology of prostatitis and its complications

 

 

The prostate is a male-specific gonadal organ and the prostate gland itself is said to be about the size of a "walnut", with bottom up, and it is close to the bladder. So the prostatitis is often coinfected with cystitis.
The prostatic apex stands downwards and the front part is inside the public symphysis. So, the patients with prostatitis often have lower abdominal pain and slight tenderness. The back of the prostate gland is close to the rectum, so long-term constipation or colitis would cause prostatitis.
The urethra goes through the prostate gland, and the stimulation of inflammation to prostate makes the gland dropsical and obese, which constricts the urethra and hinders urination.
The prostate gland is a human genital gland. It produces prostatic fluid, which is the main component of semen. The hormones secreted by prostate have a variety of physiological functions.
The seminal vesicles and prostate are anatomically close to each other. The ejaculatory duct is made up of the excretory duct of the seminal vesicle and the end of seminal duct. The ejaculatory duct goes through the prostate into the urethra. So the prostatitis is often associated with seminal vesiculitis.

 

2.The symptoms of prostatitis
1) The symptoms of acute prostatitis


Acute prostatitis is a serious bacterial infection of the prostate gland. This infection is a medical emergency. It should be distinguished from other forms of prostatitis such as chronic bacterial prostatitis and chronic pelvic pain syndrome.
Men with this disease often have chills, fever, fatigue, pain in the lower back and genital area, urinary frequency and urgency often at night, burning or painful urination, body aches, and a demonstrable infection of the urinary tract, as evidenced by white blood cells and bacteria in the urine.Acute prostatitis may be a complication of prostate biopsy.
2) The symptoms of chronic prostatitis
There are two main types of chronic prostatitis--chronic bacterial prostatitis (caused by chronic bacterial infection) and nonbacterial prostatitis.
The symptoms of chronic prostatitis vary widely. Some have no symptoms, others have very serious typical symptoms. The symptoms of chronic prostatitis are similar to those of acute bacterial prostatitis. However, men with a flare-up of chronic bacterial prostatitis tend to be less ill than those with acute prostatitis. For example, a fever (high temperature) is less likely and you are less likely to have general aches and pains.
Men with chronic prostatitis tend to have symptoms that wax and wane. During a flare-up, you can have pain and discomfort. You feel this mainly at the base of your penis, around your anus, just above your pubic bone and/or in your lower back. Pain may spread to your penis and testes. Passing stools (faeces) can be painful. You may also have symptoms from a urine infection such as pain when you pass urine, passing urine frequently or an urgent desire to pass urine. You may experience impotence (difficulty in getting an erection), ejaculation may sometimes be painful and some men have worse pain (as described above) after having sex. You may feel tired, and have general aches and pains.

 

3.The complications of prostatitis
A number of complications may result from prostatitis.
1)The complications of acute prostatitis
Complications of acute bacterial prostatitis may include:
(1) dysuria, or acute urinary retention.
(2) acute prostatiti amalgamates acute epididymitis, seminal vesiculitis, or deferentitis
(3) acute prostatitis causes testicular lymph node swollen, with tenderness
(4) acute prostatitis may cause abnormal erection, pain after sexual intercourse, impotence and premature ejaculation.
2) The complications of chronic prostatitis
Complications of chronic prostatitis are:
(1) Chronic Seminal vesiculitis:is the most common complication of chronic prostatitis. In the chronic process, the two often coexist and interact.
(2) Impotence:a common complication of chronic prostatitis.
(3) Infertility:chronic prostatitis is a very important reason to cause male infertility. Chronic prostatitis can cause reduction in secretion of prostatic fluid which lead to the liquefaction of semen. What's more, infertility could be caused by necrospermia and aspermia which result from the destruction of hazardous substance, or lack of nutrient.
(4) Urethritis:chronic prostatitis and urethritis often coexist and interact. Irritation in urinary tract is the symptom of chronic prostatitis.
(5) Cystitis:chronic prostatitis often coexists and interacts with cystitis. Bladder irritation is a noticeable symptom among the patients.
4. Tests for prostatitis
(1)Digital rectal examination:

the prostate gland is plump, enlarged, soft, with mild tenderness. If men with prostatitis for long time, the prostate would become smaller, hardened, with uneven texture and small induration.

(2) Routine examination of prostate:

Under microscope, there are more than ten white blood cells in prostatic fluid, and lecithin corpuscle is reduced. The bacterial culture of prostatic fluid is postive and it is diagnosed as chronic bacterial prostatitis. While the negtive one is the chronic nonbacterial prostatitis.
(3) Inspection with B-model ultrasound scanning:

the dividing lines of the prostate tissues are not clear and are disorganized.
